CHILD PROTECTION  
Working With Children Check – NEW FORMS
NEW FORMS    
All forms will now be available for completion on-line and by hard copy.    
There have also been some changes to the names of the forms.    
Here is an overview of the forms (mainly used by clubs/associations/branches) and link to each form.    

 VOLUNTEER / STUDENT WORKER DECLARATION

This is the form that all volunteers complete now.    
Can be completed by the volunteer at home then printed out and handed into the club.    

APPLICANT DECLARATION AND CONSENT FORM

This is the form that paid applicants/employees complete.     
This is a combination of the old PED form and Background check form.    
Can be completed by the paid employee/applicant at home then printed out and handed into the club.    

EMPLOYER REQUEST FOR BACKGROUND CHECK

This is the form the employer (club/association) fills out to background check a paid employee/applicant.    

EMPLOYER REGISTRATION FORM

All clubs and associations must be registered with the CCYP as a child related employer.    
Use this form to register your club or association if you have not previously registered.

The other new forms relate to reporting of offences and updating employer details.

INFORMATION    
The old Working With Children Check (Child Protection) forms you are/have been using will be acceptable for an interim period as the new forms are introduced.    
If you have asked your coaches/managers/referees to sign the old forms then that is still acceptable at this time and they will still be effective for 2 years.    

You can commence using the new forms as from today.  (although there do appear to be some technical difficulties occurring at this time)

All Clubs are required to: 

1. Have an identified MPIO (Member Protection Information Officer) 

2. Register as a child related employer.  Complete and submit Employer Registration Form 

3. If a Club wishes to change their Registration details, complete and submit Amendment to Employment Registration Details Form 

4. Clubs must obtain a signed Prohibited Employment Declaration (PED form) from every coach, manager, trainer and club official at the start of every season. PED Form 
 

5. If a Club is unsure if a person requires a PED Form, complete and submit Is the Position Child-Related Employment?

NOTE:  Signed forms must be kept by a Club/Association/Referee branch in a secure place for two (2) years. Forms can be requested at any time.  Failure to produce a form when requested can result in penalties for breach of the Child Protection Act.
 

6. Clubs must obtain a signed Background Check form from any person paid by the Club/Association to work with children. Working With Children Background Check Consent 

7. After a Background Check Consent form has been signed the Club/Referee branch must then fill out a Background Check Request form and send it to NSW Sport & Recreation Child Protection (CP) Unit for checking.  Working With Children Background Check Request     
The CP Unit will do a background check on the person and then issue a clearance to the Club, giving a clearance for that person to work with children. 

6. Clubs should also now request that any self employed coaches or operators hired by or paid  to coach, supervise or work with children, provide the club with a copy of their Self Employed Certificate.

7. Clubs must also report any incidents of child abuse where a child's safety or welfare was at risk.
